2) hold and present upon request:
a) a document confirming possession of health insurance as defined by the provisions of the Act of 27 August 2004 on healthcare services financed from public funds (Journal of Laws of 2019, item 1373, as amended),
or proof of having travel medical insurance with a minimum coverage amount of 30,000 euros, valid for the entire planned period of the foreigner’s stay in the territory of the Republic of Poland.
The insurance must cover all expenses that may arise during the stay in Poland due to the need for medical repatriation, urgent medical assistance, emergency hospital treatment, or death,
and the insurer must undertake to cover the cost of the healthcare services provided to the insured directly to the entity providing such services, based on the invoice issued by that entity — in the case of entry on the basis of a national visa.
